Installing Visual Basic version 5.0 on a system running Windows 95 with the OEM Service Release listed above may cause the following error message to be displayed during the system restart:
"Windows initializing device IOS: Windows protection error IOS failed to Initialize, Please restart"

In many cases this error has been caused by driver problems. You should contact your vendor to see if an updated driver is available to fix the problem.

It is also possible that the error is caused by the problem described in the following article in the Microsoft Knowledge Base:
157924  ( ) Error Message "IOS Failed to Initialize" on Boot

Microsoft has confirmed this to be a bug in the Microsoft products listed at the beginning of this article. This problem has been fixed in Microsoft Windows 98.
